Medical Examination:
Thank you for e-registration on 11-Oct-2007. You have booked your medical appointment on 07-Jan-2008 at 1300hrs.

Please report in PT attire(T-Shirt & Shorts) for your appointment with the following documents:

• National Registration Identity Card
• Medical reports, hospital appointment cards, X-Ray films and School Health Booklet
• Medical Screening Questionnaire duly signed by your parents or guardian
• Original and photocopy of all Educational Certificate(Private or Overseas Courses only)

After your medical examination, you will be required to sit for the Vocation and Psychological Test.

Please note that you would not be allowed to go through the medical examination and documentation without Medical Screening Questionnaire Form duly signed by your parent.

You are reminded to have your meal before reporting and also proper hair cut for smart card photo-taking. If you are bespectacled, please put on your spectacles and NOT contact lenses for medical examination.
